Welcome to MCPHS! The Office of Student Access and Accommodations (OSAA) is excited to welcome all our new Cardinals to the MCPHS Community. If you are an incoming student or if you are a current MCPHS student and are requesting disability-related accommodations for the first time, please follow the steps for requesting reasonable accommodations below.

Request Services

Complete a Student Request for Services Form & submit supporting documentation.

  • Students may attach documentation at the end of the SRS form or fax or scan and email supporting documentation
  • Visit our Documentation Guidelines for more details on appropriate supporting documentation

Respond to Email

Respond to the OSAA email requesting your availability for an intake appointment.

  • The email will be sent to you with the subject “Action Required: Next Steps for OSAA Accommodation Request”

Intake Appointment

Attend an Intake Appointment with an OSAA staff member.

After the intake appointment, OSAA will review your SRS, supporting documentation and self-report, and make an eligibility determination. If you are determined to be eligible for services, an Eligibility Letter will be sent to your MCPHS student email, along with information regarding next steps to implement your eligible accommodations.

Determination of reasonable accommodations is a deliberative and collaborative process between the student and the OSAA. This process includes the OSAA’s review of third-party documentation provided by the student’s qualified professional. MCPHS will consider the student’s disability, history, experience, request, and the unique characteristics of the course, program, or requirement, to determine if a specific accommodation is reasonable.

MCPHS ensures FERPA compliance and therefore all information submitted to OSAA will remain confidential.

Parent/Guardian Resources

As a parent/guardian you can help support your student by directing them to establish accommodations once they are admitted. Parents/guardians can play a crucial role in helping their high school students transition their disability-related accommodations to college. We strongly encourage parent/guardians to join us in supporting their student and to contact us early. Parents/guardians can help their student complete the request for services form, gather documentation, and set up an intake appointment.

Please note that in order to communicate with 3rd parties (I.e., parents, guardians, providers, etc.) OSAA must have a signed Release of Information on file. Submit a Release of Information Form to OSAA.
